On 13 July 2022, a Canadian Pacific Railway Grain unit train was proceeding westward when 43 hopper cars derailed at mile 97.4 of the Brooks Subdivision in Bassano, Alberta. There were no injuries.

The occurrence
On 22 February 2023, at approximately 2133 Eastern Standard Time, the empty Tacora Resources Inc. ore train W039, operated by Quebec North Shore and Labrador Railway, was travelling northward on Quebec North Shore and Labrador Railway’s Wacouna Subdivision when it passed a Stop signal without authority at Mile 128.3 at Mai Station, Quebec. There was no collision or derailment, and there were no injuries.

On , EXO commuter train 816 on the Mont-Saint-Hilaire line was proceeding eastward at Mile 62.18 on the Canadian National Railway Company St-Hyacinthe Subdivision when it struck a stopped automobile at a public crossing in Saint-Bruno-de-Montarville, Quebec. The driver of the automobile was fatally injured.

On 5 April 2023, a Canadian National Railway Company (CN) loaded unit grain train hauling 104 cars was proceeding westward on the CN Wainwright Subdivision when 18 grain hopper cars derailed at Mile 244.2. There were no injuries. The TSB is investigating.

The occurrence
On 8 August 2023, a Canadian National Railway Company (CN) freight train was travelling westward on the Wainwright Subdivision when it passed a Stop signal indication and sideswiped a westbound CN freight train that was departing the yard west of Wainwright, Alberta. As a result, two locomotives and two loaded intermodal cars on the first train derailed, while six loaded hopper cars on the second train derailed.

The occurrence
On 13 April 2023 at about 1451 Eastern Daylight Time, Canadian National Railway Company mixed freight train M 37231-13 was proceeding eastward on the south track of the Kingston Subdivision at about 43.2 mph when it passed signal 694S displaying a Stop indication at Wesco (Mile 69.4). At the same time, VIA Rail Canada Inc. passenger train P 06721-13 was travelling at approximately 45 mph westward on the south track of the Kingston Subdivision, approaching Wesco, where it was lined to cross over to the north track. Both trains stopped about 1100 feet apart.
